Surely these could vary to say anything. I'm guessing it will be watertight, so you won't be able to talk about in any public media.
Can anyone help in explaining what these are all about and what it actually stop you doing/saying?

There is a lot of myth around gagging orders. They can't undo what has already been said and done and only cover you specifically not all the people you told the whole story to prior to signing. They would be free to discuss it with whoever they chose unless they signed one too. The order might require you to hand over video evidence and ask you to remove posts on here for example.

Have been offered refund cost of fireplace which is causing major problems re heat/insulation/drafts etc and is rusting! Builders shall only refund if I sign a confidentiality agreement and don't ask NHBC to assess fireplace. Any advise?

My advice would be to not accept the offer of "compensation" to the value of the fireplace at all.
Fireplaces are not just to be viewed as a decorative centre piece to the room. They provide a very important function especially when fitted around either a solid fuel or gas fire. The issues that you raise, heat/insulation/drafts, could well be as a result of an ill fitting fireplace and would possibly mean that there could be concerns over "spillage" of carbon monoxide, which as you should be aware is a matter that should be addressed immediately.
The very fact that the builder is wanting you to sign an agreement that would prevent you from taking up the issue with the NHBC should be ringing alarm bells for you.
Tell the builder that you want a full independent inspection to be made by a specialist in this field.

If you were a house builder would you want the faults in the homes you have built and you have compensated your customer for publicised? Of cource not!
They don't want the NHBC to know either otherwise they will increase the premiums they charge the house builder for the warranties issued as the builder would represent more of a risk.
Finally and most important they dont want anyone else to find out that compensation was paid!
All the major house builders claim they have a policy of NOT paying compensation, but when push comes to shove they all do in the end!
The last thing all house builders want is bad publicity. That is why they have the HBF to look after their interests.
Nothing gets things put right quciker and compensatation than putting a sign on front of your house saying
"Dont buy a home on this development without talking to me first"
